Step 1

In the address part of the application, it is necessary to indicate to which registering tax authority it is submitted. Often this is the same inspection in which the company is registered at its legal address. But there may also be a separate registering inspection (in Moscow it is MINFS-46).

You can clarify this issue on the website of the Federal Tax Service of Russia using the search service of the Federal Tax Service Inspectorate at the legal address. If a separately registering inspection is indicated in the results, then you need to contact it.

In this case, the state duty is paid to the registering inspectorate and according to its details for making changes to the Unified State Register of Legal Entities.

Step 2

The section devoted to information about a legal entity is filled out on the basis of its constituent documents, a certificate of state registration of the company and the assignment of a TIN and KPP to it.

The constituent document of the company under the current legislation is its charter, and certificates of state registration of an enterprise and the assignment of a TIN and KPP to it are issued by the tax office after the registration of the company.

If these documents are lost for some reason, you will first have to take care of obtaining duplicates.

Step 3

In the section on the reason for the change, the relevant items are marked with a "V". Next, you need to fill out only those sheets that are dedicated to the necessary changes. In the rest, you do not need to write anything.

In some cases (for example, when the composition of the founders of the company changes), copies of decisions confirming the fact of changes are attached to the application.

If we are talking about changes in the Unified State Register of Legal Entities that are not related to amendments to the constituent documents, information about them is certified by the applicant's signature. This role is played by the general director (or other first person of the enterprise), and his signature is certified by a notary.
