A certificate from the place of work on the average earnings is drawn up on a written application of the employee, who may need it, for example, to receive benefits in connection with dismissal. In order to arrange it correctly right away, pay attention to some important nuances.
Instructions
Step 1
In the upper left corner of the certificate sheet, there should be a corner stamp (if any), if the enterprise does not have one, then the inscription "The enterprise does not have a corner stamp" is made on this place, which is certified by the signature of the head and the decoding and seal of the enterprise. The text of the certificate must contain the full name of the organization, TIN, registration number in the Social Insurance Fund of the Russian Federation, TIN of the employee, his last name, first name and patronymic - in full. Do not forget to put down the outgoing number and the date of registration of the issued certificate.
Step 2
The period of work at the enterprise, indicating the date of admission and dismissal, is filled in in accordance with the entries in the work book.
Step 3
The certificate must reflect the salary accrued for the last three months before dismissal. If the months are replaced, then indicate the reason for this and the reason.
Step 4
Fill in the boxes indicating the accrued wages for the last 12 months prior to dismissal. Specify for each month the planned number of working days and actually worked, the reason if these values do not match.
Step 5
The certificate must reflect the nature of the work: summarized accounting of working hours, part-time or working week. Indicate from what time and to which employee worked part-time or weekly, how many hours a day or days a week, order number and date.
Step 6
When calculating the average earnings, be guided by the methodology approved by the Decree of the Ministry of Labor and Social Development of the Russian Federation No. 62 of 12.08.2003. It stipulates cases when time and wages accrued for it are excluded from the calculation period. In particular, this may be in cases where the employee received temporary disability benefits or maternity benefits, or he was not able to work due to the fault of the employer.
Step 7
Write the calculated average salary in numbers and in words with a capital letter.
Step 8
Sign the certificate with the chief accountant and the head of the enterprise, sign the contractor, give it a decryption, indicate the contact phone number.
