To officially register your business as an individual entrepreneur, an individual must fill out an application for registration in the form p21001. The form of this document is approved by Appendix No. 18 to the Decree of the Government of the Russian Federation No. 439 of June 19, 2002.

Instructions
Step 1
On the first page of the application, indicate the name of the registering authority at your place of residence and its number, enter the last name, first name, patronymic, date and place of your birth in accordance with the identity document, Completely write the address of your permanent residence (postal code, region, city, locality, street name, house, building, apartment number) and contact phone number, fax number (if any).
Step 2
On the second page of the document, enter the details of the identity document (series, number, date of issue and the name of the issuing authority). In the case of opening a company that is an individual entrepreneur, a person who has not reached the age of majority, indicate from the list provided the basis for acquiring the legal capacity of a citizen, write the type and details of the document confirming this right (number, date and name of the authority that issued it). If the company is opened by a foreign citizen, indicate the details of the international document, the type and data of the document that gives the person the right to reside in the territory of the Russian Federation.
Step 3
Indicate on sheet A of the application the code of the type of economic activity in accordance with the All-Russian Classifier of Types of Economic Activity and the name of the type of economic activity.
Step 4
Pay the state fee in any bank of the Russian Federation in the amount of four thousand rubles, a receipt confirming the payment made, with the signature of the cashier and the seal of the bank, attach to this application.
Step 5
On the third page, confirm the accuracy and completeness of the information provided in the document with your personal signature, enter the taxpayer identification number, if available. Verify your signature with a notary, who, in turn, will sign the application in the appropriate field, certify it with a seal and indicate his taxpayer identification number.
Step 6
Attach the required package of documents to the registration application, depending on your status, submit to the registering authority, which will write a receipt for its receipt.
