Instructions
Step 1
To obtain a medical license, you need a large list of documents by order of the Ministry of Health No. 142 dated April 29, 1998 "On the list of types of medical activities subject to licensing." It is in this legislative act that the need for a license for a beauty salon is indicated.
Step 2
Collect the following documents:
- a copy of the certificate of state registration of the organization as a legal entity;
- copies of constituent documents - regulations, articles of association and constituent agreement;
- a copy of the letter or certificate of registration with the tax police;
- copies of documents for the right to own or use the premises for the beauty salon;
- copy (s) of all licenses you already have;
- conclusion from the TsGSEN of your city (original and copy);
- the conclusion of the fire service on the compliance of the premises for engaging in cosmetic activities with fire safety requirements (original);
- explanatory note - a brief description of the proposed medical activity.
- copies of certificates of your professional suitability in a beauty salon: diploma of education, certificates of specialization, etc. etc (with the presentation of the original, if the copy is not certified by a notary);
- bank payment order or a copy of the license fee payment receipt.
Step 3
You are legally responsible for all information provided in the documents. All the documents that you collected to obtain a license will be taken from you according to the inventory, and a copy of it with the date of receipt of the documents should be in your hands (it will be sent to you after the documents are received). This will be an additional guarantor of your actions.
