Indicate the code of your company in accordance with the All-Russian Classifier of Objects of Administrative-Territorial Division.

Specify the organization code in accordance with the All-Russian Classifier of Enterprises and Organizations.

Enter the code of the organizational and legal form of your organization in accordance with the All-Russian Classifier of Organizational and Legal Forms.

Enter the full registration address of your company (postal code, region, city, district, street, house, building, apartment number).

Write down the number of insured persons employed in your organization. Indicate the average number of the company.
