The statement of payment of insurance contributions for compulsory pension insurance is filled in by employers and is submitted once a year. The ADV-11 form was developed and approved by the Decree of the Government of the PF of Russia N 192p. Also, this document is transferred to the local PF in intermediate periods, that is, quarterly, together with the forms SZV-4-1 and SZV-4-2.

Instructions

Step 1

Enter your company code in accordance with OKPO. Write the code of the enterprise assigned by the Pension Fund of the Russian Federation when registering with this body. Enter TIN, KPP of the company. If the organization's OPF is an individual entrepreneur, indicate only the TIN.

Step 2

Briefly write the name of the organization. For example, LLC "Prodservice" or IP Morozova P. G. Indicate in Arabic numerals the reporting year for which the statement is filled out. Enter the date, month (in full), year, as of which the document was drawn up.

Step 3

Enter the number of packs of previously filled out forms SZV-4-1 and SZV-4-2. Indicate the number of insured persons, that is, the number of personnel (employees) with pension certificates (SNILS), performing obligations under an employment contract with an employer, that is, with a company that pays insurance premiums for the future pension of employees.

Step 4

Indicate the code of the main tariff (this includes the majority of employees), additional tariff (it includes certain categories of specialists, the list of which is spelled out in the legislative acts of the Pension Fund of the Russian Federation).

Step 5

Further information is entered on the basis of the data specified in the forms SZV-4-1 and SZV-4-2. Fill out the table in which you enter the amount of arrears on insurance premiums at the beginning of the reporting year. Separately write the amount of the calculated contributions, which are transferred under the insurance part of the pension, the funded one (only for employees born before 1967) and the amount of contributions calculated at the additional rate.

Step 6

The calculation of insurance premiums is recorded in the following table. In the first column, write a tariff sign. When applying the maximum rate, put "M". If you are calculating contributions on a regressive scale, please indicate “P”.

Step 7

Enter the paid insurance premiums for the reporting year in the table. Indicate in separate lines the amount of contributions that are listed for the insurance part of the pension, the funded one (for persons not older than 1967), as well as the amount of deductions for employees whose categories belong to the additional tariff.

Step 8

By subtracting from the amounts of arrears in insurance premiums at the beginning of the year, the contributions paid in this period, find the amount of arrears in contributions to the Pension Fund of the Russian Federation for the insurance, funded part of the pension and contributions calculated at the additional rate.

Step 9

If there are arrears for previous years, add and fill in the columns in the table of contributions arrears at the beginning of the period. When an overpayment occurs, similarly enter the amount of insurance premiums, and they must be indicated with a "-" sign.

Step 10

Certify the statement with the signatures of the chief accountant of the company, director of the company (indicating their personal data, positions held).
