The Unified License Register as an automated system for collecting, accumulating and recording data related to the licensing of entrepreneurial activities was created in Ukraine in 1997. Any organization or citizen of Ukraine can apply for the information contained in the register.
Instructions
Step 1
Information from the Unified License Register is provided to users by mail, as well as using electronic means of communication. If you are not a representative of the authorities, you will have to pay a state fee for services in the established amount.
Step 2
If you want to check if the entrepreneur was actually issued a license, ask him for a copy to check the availability of this document by number. If you do not know whether this license was issued or not, then in this case you can send a request, indicating only the name of the organization
Step 3
Indicate in the request (depending on what information you already have): - the name of the organization; - the license number; - the address and form of ownership of the organization In the request, you must also indicate the name of the applicant, his address and identification code.
Step 4
Send a request for information from the Unified License Register in writing to the State Committee of Ukraine on Regulatory Policy and Entrepreneurship at the address: 01011, Kiev, st. Arsenalnaya, 9/11.
Step 5
Depending on the type of request and the amount of information found upon request, you will receive the information you need within 30 days after you pay the invoice sent to you by mail. If you are a representative of government agencies, all information will be provided to you as soon as possible.
Step 6
Apply with a similar request to the relevant ministry of Ukraine. So, for example, if you want to know if the university where you are going to study has a license, send an appeal to the Ministry of Education. And if you are going for treatment, ask if the clinic has a license issued by the Ministry of Health of Ukraine.
