Step 1
To do this, you need to come to the district Department of Social Protection of the Population and fill out a special application form.

Receive an application form in the appropriate window. Turn the questionnaire over to the other side. Read the rules (instructions) for filling out and filling out the application form.

Take a blue or black ballpoint pen. Complete the application form in accordance with the instructions for completing in legible block letters.

In the application form, an individual must indicate, in addition to full personal data (last name, first name, patronymic, actual residence and registration address, date and place of birth, telephone number, etc.), also information about changing the last name (if any), number of the identity document.

Indicate the place of work in full, and if the allotted place for the name is not enough, shorten it so that it is clear what kind of organization we are talking about. Use generally accepted and well-established abbreviations (RF, Moscow region, Southern Administrative District of Moscow, etc.). Enter your position in the appropriate box. If you do not work, put a tick in front of this item and leave the fields "place of work", "Position" empty.

Migration card data is filled in by foreign citizens and stateless persons legally located in the Russian Federation. They also enter into the questionnaire the data of the document confirming your right to reside in the Russian Federation, etc.

List in the questionnaire all the documents that are attached to the application - this can be a certificate of family composition, a certificate of income (from work or from a bailiff if you receive alimony or other compensation payments), certificates of disability, etc.
