To make settlements with funds that are in bank accounts, it is necessary to issue a card with a stamp and sample signatures of an individual entrepreneur or officials of a legal entity who have the right to sign documents. The rules for issuing a bank card are clearly regulated by the relevant instructions of the Central Bank of Russia.

Step 1

Use a black typewriter (you can use a computer printer) or a black, blue, or purple ink pen to fill out a bank card. A facsimile signature cannot be used to fill out the card.

Step 2

In the "Account holder" field, enter the full name of the legal entity (separate subdivision) in accordance with the certificate of state registration. If the card is filled in for an individual, indicate your last name, first name, patronymic, date of birth, details of an identity document. If you are in private practice (notary, lawyer), indicate the type of activity.

Step 3

In the "Location (place of residence)" field, indicate the address of the permanent executive body of the legal entity or its separate subdivision, the address of the actual place of residence of the individual (individual entrepreneur).

Step 4

In the field Tel. N”indicate in brackets the code of the locality, and then the phone number of the organization or individual entrepreneur.

Step 5

In the "Bank" field, enter the full name of the bank or subdivision of the settlement network of the Bank of Russia where you open a bank account.

Step 6

Do not make any entries in the Bank Mark field. Here, after assigning a number to the bank account, a note is put on the acceptance of the card by the person who has been granted such a right. Also leave the field "Other marks" blank (it is filled in by the credit institution).

Step 7

On the back of the bank card, in the “Short name of the account holder” field, indicate the short name of the legal entity recorded in the registration certificate, and in its absence, the full name. An individual client puts down his last name, first name and patronymic, as well as an indication of the type of activity.

Step 8

In the "Position" field, indicate the positions of persons who have the rights of the first or second signature. If you are filling out the card as an individual, leave this field blank.

Step 9

In the "Last name, first name, patronymic" field, indicate the full last name, first name, patronymic of persons vested with the rights of the first or second signature.

Step 10

In the field “Signature sample”, the signature of persons who have the rights of the first or second signature is put in front of their surname. In the "Date of filling" field, indicate the date when the bank card was filled out.

Step 11

In the field “Client signature”, the head of the legal entity must put down a handwritten signature, and in the case of a card for an individual - an individual entrepreneur or a person engaged in private practice.

Step 12

In the field “Term of office of persons temporarily using the right to sign”, indicate the term of office of persons established by the client's administrative act.

Step 13

In the "Sample print print" field, put a sample print (if available). The print must be clear.

Step 14

If you wish, certify the authenticity of the handwritten signatures of persons entitled to the first or second signature by a notary public. A bank card can be issued without notarization in the presence of an authorized bank employee.
