Instructions
Step 1
Contact the center for social protection of the population to block the lost social card. This will prevent illegal use of your document.
Step 2
If your card is attached to a bank account, then you should also contact the bank and report the loss of the document. Otherwise, funds may be illegally withdrawn from your account and cannot be recovered. Despite the fact that OJSC Bank of Moscow has a customer support service, which can be called by the round-the-clock telephone (495) 728-77-88, to block a lost social card, you must appear in person with a passport or other document proving your identity.
Step 3
Write a statement to the bank for blocking a social card with a detailed description of the circumstances of the loss. Receive notification that this procedure has been completed. It is also advisable to ask for an account statement to determine whether the fraudster managed to use your funds.
Step 4
Visit the district department of social protection of the city's population and go through the procedure for obtaining a social card again. Fill out the application form, in which, in a special line, mark that you want to restore the lost card.
Step 5
Provide your RUSZN employee with your 3x4 photo; you can also take a picture for free right in the office. Receive a tear-off coupon of the application form for restoration, which must be kept until the card is received.
Step 6
Get a temporary social card that will allow you to use public transport at a reduced rate until your social card is returned. As a rule, it takes about a month to issue a new card, while it is integrated with your personal bank account, which contains your compensation, benefits or pensions.
