All organizations, at least somehow related to alcohol, whether it be the production of alcoholic beverages or alcohol-containing cosmetics, the owners of cafes, wine glasses or huge supermarkets - everyone must register and declare alcohol, in other words, submit declaration reports to the Federal Alcohol Regulation Agency (FSRAR).

Appendix 1. Declaration on the volume of production and turnover of ethyl alcohol. To be filled in by organizations engaged in the production of alcohol, as well as circulation. In any quantity.
Appendix 2. Declaration on the scope of use of ethyl alcohol. To be completed by organizations that use alcohol in the production of something - cosmetics or medicines, for example.
Appendix 3. Declaration on the volume of production and turnover of alcoholic and alcohol-containing products. The name speaks for itself. This declaration is filled out by enterprises - producers of alcoholic beverages of any strength.
Appendix 4. Filled in by enterprises - producers of alcoholic beverages of any strength, purchasing alcohol, using it as a raw material, auxiliary material.
If you are not a manufacturer of alcohol and alcoholic beverages in general, then you need to hand over Appendices 5, 6, 7 to Order PAP No. 231 of 23.08.2012. This is the purchase (how much and from whom you bought), sales (to whom what kind of alcohol was sold) and turnover (sold and bought by manufacturers). We start with appendix # 7 - sales volume, then appendix # 8 - purchase volume. We print out both reports and transfer the data from them to Appendix No. 5 turnover volume.
Shipping companies fill out and submit Form 9.
Appendix 10. Declaration on the use of ethyl alcohol production facilities. To be filled in by organizations engaged in the production of alcohol, beer drinks, poiret, mead from ethyl alcohol.
Appendix 11. About the volume of retail sales of alcohol (except beer, etc.) To be completed by retailers. For beer, fill in Appendix 12.
The declaration consists of a title page and a declaration form.
