Step 1
Take the form of cash on delivery from the employee of the post office in the appropriate window (sending and issuing mail). In the first line (arrow 1), write the amount of the declared value in capital letters. In the second line (arrow 2), also in capital letters - the amount of cash on delivery. In the "To" field (arrow 3), indicate the last name, first name and patronymic (without abbreviations, in the genitive case) of the person to whom you are sending the parcel. Further, the index and full address (arrows 4 and 5), including the republic, district, city, street, numbers: houses, buildings and apartments. For example: the Komi Republic, Priluzsky district, with. Obyachevo, st. Svobody, 45. Depending on the place of residence, in some cases it is enough to indicate only the city, street, house number, building number, apartment, and when sending to small villages - the name of the village, last name, first name and patronymic of the addressee.
Step 2
After filling in the data about the addressee, go to the indication of your data. In the line "From whom" (arrow 6) enter your last name, first name and patronymic also in the genitive case. On the next line (arrow 7), indicate the postcode and address that fully matches the one indicated in your passport on the "Residence" page. In the line below - sign, thereby confirming that there is nothing prohibited in the shipment, and that you are familiar with the packaging requirements.
Step 3
At the bottom of the form, located in the frame, indicate the amount of the declared value and the amount of cash on delivery in numbers, both amounts must be the same. Then enter the data "To" again: last name, first name, patronymic without abbreviations and in the genitive case, full address and index. All other blank fields are filled in by an employee of the post office. After receiving a notice of receipt of a parcel in his name, the addressee will have to pay the amount indicated in the form (cash on delivery) in order for him to receive the parcel.
