- - certificate of making an entry in the Unified State Register of Legal Entities about a legal entity;
- - a document confirming the state registration of a legal entity;
- - a certificate from the tax service on the registration of a legal entity;
- - lease agreement for the address of the massage parlor.
Instructions
Step 1
Collect the founding documents of your organization. The package must consist of a charter, a memorandum of association, if there are more than two founders. Attach a document confirming the state registration of a legal entity, a certificate from the tax service on the registration of a legal entity.
Step 2
You will also need a certificate of making an entry in the Unified State Register of Legal Entities about a legal entity (if your company was registered before 01.07.02). If during its existence the charter of a legal entity has been changed or supplemented, it is mandatory to have a certificate of its changes.
Step 3
Conclude a lease agreement for the address of the massage parlor or submit documents for the ownership of the premises.
Step 4
Prepare documents for the entire staff. Be sure to confirm the availability of higher or secondary specialized education of your employees, the level of their training that meets the requirements for the implementation of these services.
Step 5
The person performing the duties of the head of the organization must submit documents confirming the presence of a licensee or an individual authorized by him a document on higher special education. At the same time, the management must have at least 5 years of experience in licensed activities.
Step 6
Make sure that employees of the company receive advanced training at least once every 5 years. Do not forget about the original sanitary and epidemiological report.
Step 7
Document the availability of the required material and technical base and compliance with the organizational and technical conditions.
Step 8
Sign a contract with a company that will carry out maintenance of your medical equipment. Attach a copy of this organization's license.
To all of the above, you should add a letter from the State Statistics Committee.
