Step 1

Entrepreneurs are obliged to create work books for each employee whose employment relationship lasted more than five days.

Before making another entry in the employee's work book, the entrepreneur must indicate his full name as the title: "Individual entrepreneur Ivan Ivanov."

Step 2

Then it is necessary to indicate in the appropriate columns the ordinal number of the entry, the date of its entry (strictly in Arabic numerals in the format "dd.mm.yyyy"), a record of employment with an indication of the position with exactly the same wording as in the employment contract and the order of admission for work and the output of this order (name, can be abbreviated, number and date).

Subsequent entries are made in the same format: about transfer to another position, dismissal, etc.

Only the dismissal record is certified by the seal and signature.

Step 3

If the employee does not have a work book, the entrepreneur is obliged to create this document for him. At the same time, he has the right to collect the cost of the form from the employee or withhold this amount from his salary.

All the necessary information for the title page is filled in on the basis of documents proving the identity of the employee and confirming his education, qualifications, etc.
