An entrepreneurial type of activity that is carried out on the territory of Russia is considered legal only if there is a certain permitting document. Such a document is a license. In turn, a license is also required to provide psychological assistance or other medical services.
Instructions
Step 1
Collect the required set of documents:
- documents that indicate the compliance of employees with the required professional level for a specific type of activity;
- a document confirming the presence of a working space and compliance with all the required sanitation standards. These documents must be certified by a notary;
- certificates for the use of technologies;
- passports for instruments, medical equipment and vehicles (if any);
- constituent, registration documents;
- decision from the sanitary and epidemiological station;
- Goskomstat codes;
- documents confirming the qualifications of the head of this legal entity (certificate, diploma, work book);
- a document confirming the ownership or lease of the premises;
- confirmation of the state of technical equipment in the working room, expert opinion.
Step 2
Write an application for a license. An entity wishing to carry out certain types of medical activities that must be licensed independently or with the help of a specific person authorized by him must apply to the special licensing authorities with a written application in the prescribed form for the issuance of the necessary license.
Step 3
Please include the following information on your application:
- information about the business entity (applicant). Its name, location, bank details, identification code (of a legal entity). Then write the full name and identification number of the individual, as well as mark his passport data;
- the type of business (provision of medical care) for which you intend to obtain a license.
Step 4
Pay the required amount (fee) to process your application. Save your receipt.
Step 5
Submit all collected documents along with the application and receipt to the appropriate government agency.
