Start filling out the first sheet by indicating your TIN in the appropriate column, then enter the year for which you are reporting. Next, you fill in the number of the division of your tax inspectorate, then write your data in block letters, and even further the OKVED number. The third page begins by filling in the tax rate data. This should be followed by the amount of income and expenses for the entire year. The following columns are filled in depending on the selected taxation regime ("income" or "income minus expenses") and vary greatly.
