The form for writing off goods is filled out if damaged or stolen products cannot be properly processed and at the end of the current year its value is added to the total shortage. Of course, writing off such goods will not bring the company any income, but it will help to detect the problem.
Instructions
Step 1
Type in the very top left corner of the document: "Form # 13". Below type its name: "Product write-off form" or simply: "Product write-off".
Step 2
Make a table. In the first column, in the "header" write: "Product name" or "Product number". In the second column, on the very first line, type: "Product Description".
Step 3
Write in the third column, in the "heading": "Quantity of products", in the fourth: "Selling price". Next, in the fifth column, on the very first line, write: "Product amount". This refers to the cost of production in accordance with its quantity.
Step 4
Fill in the header in the sixth column: Stolen / Damaged. In this column, you will need to enter data on why each individual product is being written off.
Step 5
Enter the data into the table. In this case, use various documents confirming the receipt of products (invoices, quality certificates).
Step 6
At the very bottom of the table, type: "Total Debited". And then indicate how many goods were written off in total and what the total amount is. In order to calculate this value, it is necessary to add up all the values in the fifth column.
Step 7
Indicate below the table who wrote off the goods and who prepared this document.
Step 8
You can record all other write-offs of goods using this form. With the help of it, you can take into account not only the cost of damaged goods, but also those products that disappeared from empty boxes found at the enterprise. In addition to all this, in the same document, you can record the use of various goods in the needs of the company.
Step 9
Mark in the form of a footnote what documents you used when drawing up the form for writing off goods. To do this, at the very bottom of the document, list similar documentation.

Date the form.
